Three kinds of information

It helps to keep three things apart.

• Website information. What this policy covers. It is what you send us through the request form or by email, plus the ordinary technical data your browser sends when you visit.
• Nursing care information. Your intake and consent forms, health and medication history, and the notes kept during in-home care. These are collected on a secure, HIPAA-capable form service or on paper, kept by the nurse as your nursing record, and covered by the privacy notice you receive before your first visit.
• Telehealth care information. Your intake answers, visit notes, prescriptions, lab results and payment details for physician services. These are collected on the telehealth platform by Heally Health, protected under HIPAA, and covered by the privacy notice you receive when you start care.

The request form

The "Request Concierge Care" form asks for your name, phone number, email address, your town or zip code, who the care is for, the kind of help you are looking for, timing, and the best time to call. It deliberately does not ask for medical details, and we ask you not to include any. What you send is delivered to the nurse so she can call you back. It is not stored on this website.

What we collect here

• What you give us. Your name, email address, phone number, and anything else you choose to include when you contact us or ask to be called back.
• What is collected automatically. Pages viewed, time of visit, the site that referred you, your browser and device type, and an approximate location derived from your IP address.
• Cookies and similar technologies. Small files that keep the site working and help us measure traffic. You can block or delete them in your browser settings, and the site still works without them.

We do not ask for health details through this website, and we would rather you did not send them by email. Health details belong on the intake forms you complete before your first visit, or on the telehealth platform, where that information is protected.

How we use it

• To run the site, to keep it secure, and to make it better.
• To answer you when you get in touch, and to call you back when you ask for care.
• To understand which pages bring people here, in aggregate.
• To meet legal obligations, and to protect clients, the nurse, and the public.

When we share it

• Service providers. Companies that host the site, deliver form submissions and email, or measure traffic, acting on our instructions and not for their own purposes.
• The medical group and the telehealth platform. When you book a telehealth visit, so that Heally Health can arrange your care.
• Your surgeon or physician. With your permission, the nurse coordinates with the office whose orders she follows. That is part of nursing care, not website information.
• Legal reasons. Where the law requires it, or to protect the rights and safety of clients, the nurse, or the public.
• A change in the business. If At Home Wellness is ever part of a merger, acquisition, or sale of assets, information can move with it, still subject to this policy.

We do not sell your personal information. Some state privacy laws treat analytics cookies as "sharing." If you want that stopped for your visits, email athomewellnessrn@gmail.com or use your browser's opt-out signal, which we honor where the law requires it.

Health information and HIPAA

Protected health information created during care is used and disclosed only as HIPAA and state law permit: for treatment, for payment, for health care operations, where you authorize it, and where the law requires it. Nursing records are held by the nurse. Telehealth records are held by Heally Health and the telehealth platform. None of them are held on this website.

Requests about your records should go to the nurse or the care team rather than through this site, so that your identity can be verified before anything is released.

How long we keep it

Website information is kept only as long as it is useful for the purpose it was collected for, and is then deleted or anonymized. Nursing and medical records are kept for the period required by the law of the state where care was provided, which is usually several years.

How we protect it

We use encryption in transit, access controls, and vendors chosen in part for their security practices. No website or service can promise perfect security, so we will not pretend otherwise. If something happens that affects you, we will tell you where the law requires it.

Your choices and your rights

• Browse without giving us anything. Nothing on this site requires an account.
• Unsubscribe from any marketing email with the link in the message, or by asking us.
• Reply STOP to any text message to stop texts.
• Ask what we hold about you, ask us to correct it, or ask us to delete it.

Depending on where you live, you may have further rights, including a copy of your information and the right to appeal our decision on a request. We will not treat you differently for exercising any of them. Email athomewellnessrn@gmail.com and say what you want. We may need to verify who you are first, and for care records we will point you to the nurse or the care team.

Children

This site, and the care behind it, are for adults, or for adults arranging care for a family member. We do not knowingly collect information from anyone under 18 through this site. If you believe a child has sent us something, email athomewellnessrn@gmail.com and we will delete it.

Other companies' sites

Links here can take you to the telehealth platform, the intake form service, a pharmacy, a laboratory, or another company. Once you are there, their privacy policies apply instead of this one, and they are worth reading.

Where we operate

At Home Wellness serves clients in the United States, and information is processed here. If you visit from another country, you are sending your information to the United States.

Changes to this policy

If this policy changes, the new version appears on this page with a new date at the top. If a change materially affects how we handle your information, we will say so more directly than a quiet edit.
